How do I become a titan?

I've heard its an Orc class, but so far I've tried I only got Raider > destroyer and
Monk > Tyrant..

Its Raider >> Destroyer >> Titan

Titan is your last class change.

By the way is a Dual hand sword or pole arm better for it? Because there is Crush of Doom for Dual Hand Swords while my Pole arm mastery is level 50 while my sword mastery is only level 23..

You should have 2-handed sword/blunt mastery at the same level as you'r polearm mastery. Poles are good if you are in a AoE party, swords/blunts for single targeting.
